Contacting our office

Messages left on our office phone line will be answered within 3-4 working days. Diane's practice is a small, one-person operation. She has part-time help at the front desk, therefore all calls are automatically routed to voicemail.

Medication refills

Please allow 5 business days for controlled substances to be sent to your pharmacy and 3 business days for all other medications. When requesting a refill, please indicate your preferred pharmacy.

Cancellation policy

If you find that you can no longer keep your appointment, please cancel at least 24 hours in advance. By doing so, you enable us to schedule another appointment for that time. A failure to cancel more than 24 hours in advance is subject to a $50 failure-to-cancel fee for returning patients, and a $100 failure-to-cancel fee for initial patients. 

Requesting records

We are happy to send copies of records of your visit(s) to your physician, insurance provider, or for disability claims. Please fill out our "Request for Records" form, and your request will be processed within 30 days. Please have all relevant addresses or fax numbers available. If paperwork needs to be filled out by Diane, there may be a charge incurred. Fees may be incurred for completing FMLA paperwork, social security forms, extensive school forms, or diagnosis letters.
